Support Coordination

Bankstown women’s health centre can assist you with all the complex forms, applications and the correct combination of support for you, and the impacts on your daily life such as, daily living activities, self-care, community connections and participation. Bankstown women’s health Centre can also assist you with Housing NSW applications, Centrelink and National Disability Scheme Access applications. We can assist you to understand your plan, connect you to supports, Design your support approaches and establish your support. We offer Crisis planning and prevention, coaching and action

There are three levels of support co-ordination that can be included in your plan:

  1. Support Connection – this support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals.
  2. Support Co-ordination – this is a co-ordination of support and will assist you to build the skills you need to understand, implement and use your plan. A support co-ordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in your community.
  3. Specialist Support Co-ordination – this is a higher level of support co-ordination. It is for people whose situations are more complex and who need specialist support. A Specialist Support Co-ordinator will assist you to manage challenges in your support environment and ensure consistent delivery of service.

This can be included in your capacity building supports NDIS funds.
